At a Glance
What is VIP?
VIP stands for:
Vasoactive Intestinal Peptide also historically called:
Vasoactive Intestinal Polypeptide VIP is a naturally occurring 28-amino-acid neuropeptide found throughout the nervous system, gastrointestinal tract, lungs and other tissues.
Its human sequence is:
HSDAVFTDNYTRLRKQMAVKKYLNSILN-NH2 The FDA’s substance record identifies aviptadil as the synthetic human VIP peptide and lists the same 28residue amidated sequence. FDA also explicitly warns that having an FDA UNII substance record does not mean the substance is FDA approved. ( FDA substance record ) VIP has broad biological effects involving:
• vascular tone
• pulmonary blood vessels
• airway smooth muscle
• gastrointestinal secretion
• neuronal signaling
• circadian biology
• immune-cell signaling
• inflammatory signaling
• epithelial biology
It acts primarily through:
VPAC1 and:
VPAC2 receptors.
The older Navaro audit was correct that VIP has genuine human evidence, but that evidence is highly dependent on the exact route, formulation, indication and dose .
What is Aviptadil?
Aviptadil is synthetic human VIP .
This is a much cleaner identity relationship than many peptides in the Navaro library.
Unlike:
TB-500 vs full-length Tβ4 or:
GHK-Cu vs AHK-Cu aviptadil is intended to reproduce the human 28-amino-acid VIP sequence. FDA’s substance database recognizes aviptadil as an ingredient substance corresponding to human VIP . ( FDA substance record) But identity does not solve the route problem The following are still very different clinical exposures:
• continuous IV aviptadil
• inhaled/nebulized aviptadil
• intranasal VIP spray
• intracavernosal aviptadil + phentolamine
• community subcutaneous VIP
• modified or extended-duration VIP analogues
The fact that they contain or derive from the same 28-amino-acid peptide does not make their doses interchangeable.
The Route Distinction Is Critical
Intravenous VIP / Aviptadil
Real human pharmacology and randomized clinical-trial evidence exists.
Human IV studies include:
• classic pharmacokinetic studies
• physiologic infusion research
• acute respiratory-distress programs
• randomized COVID-19 trials
This is a genuine human evidence category.
Inhaled / Nebulized VIP / Aviptadil
Real human evidence exists.
This includes research in:
• pulmonary hypertension
• sarcoidosis
• COVID-19 pneumonia
• other pulmonary conditions
Again, the findings are route and disease specific.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 2
Intranasal VIP
Human evidence exists, but it is much weaker .
The best-known literature comes from a small 20-patient open-label CIRS-WDB clinic study , not from a large randomized intranasal development program. ( 2013 CIRS study PDF )
Subcutaneous VIP
This is the major evidence gap.
Community sites now publish SC protocols involving approximately:
50-200 mcg per administration but I did not identify a completed human SC dose-finding program establishing:
• optimal dose
• frequency
• pharmacokinetics
• clinical benefit
• maximum dose
• long-term safety
for ordinary unmodified SC VIP .
Critical rule IV , inhaled, intranasal and intracavernosal human evidence cannot automatically be converted into a subcutaneous wellness protocol.
How Well Studied Is VIP / Aviptadil?

EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 3
This is not a:
“no human research” compound.
It is also not a compound with a clinically validated general:
immune / CIRS / wellness SC protocol.
What May It Help With?
Areas With Genuine Human Evidence
Pulmonary Vascular Effects Small human studies show that inhaled VIP can acutely alter pulmonary vascular resistance and pulmonary arterial pressure.
Pulmonary Hypertension Small pilot research reported improvements in pulmonary hemodynamics and exercise capacity with inhaled VIP .
Sarcoidosis A small open-label study demonstrated measurable immunologic changes in the lungs after inhaled VIP .
COVID-19 Pneumonia Human randomized research exists by both:
• IV
• inhaled
routes.
The results differ substantially by study and route.
Erectile Dysfunction Aviptadil has a legitimate regional pharmaceutical use when combined with phentolamine and injected intracavernosally.
That is a real authorized product context.
What Is Not Established?
VIP / aviptadil has not been clinically established as a general SC treatment for:
• chronic inflammation
• “immune balancing”
• autoimmune disease
• CIRS
• mold illness
• fatigue
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 4
• brain fog
• circadian optimization
• gut healing
• anti-aging
• athletic recovery
• general respiratory support
• neuroprotection
• infection prevention
• longevity
Likewise, controlled human research has not established that:
50 mcg SC or:
100 mcg SC or:
200 mcg SC produces predictable systemic clinical benefits.
Human Pulmonary-Hypertension Evidence
2003 Primary Pulmonary Hypertension Pilot
One of the strongest reasons VIP became interesting as a pulmonary drug was a small 2003 study in severe primary pulmonary hypertension.
Eight patients were studied.
A single:
100 mcg inhaled VIP dose reduced mean pulmonary arterial pressure by approximately:
10 mmHg and improved cardiac output.
Patients then received:
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 9
200 mcg/day inhaled VIP in four divided inhalations. ( JCI study) After approximately 12 weeks, the investigators reported:
• lower pulmonary arterial pressure
• increased cardiac output
• substantially lower pulmonary vascular resistance
• improved six-minute walk distance
The reported improvement in six-minute walking distance was approximately:
113 meters at 12 weeks and remained substantial at longer follow-up.
Important limitations This was:
• only eight patients
• uncontrolled
• not blinded
• conducted in an older pulmonary-hypertension treatment era
• not an SC trial
Correct evidence classification Strong physiologic signal in a tiny uncontrolled inhaled human study not:
proof that VIP is an established PAH treatment
2008 Pulmonary-Hypertension Study
A later study evaluated:
20 patients with different forms of pulmonary hypertension.
Each inhaled:
100 mcg aviptadil once during right-heart catheterization.
The study found a small, temporary but statistically significant selective pulmonary vasodilator effect.
Only:
6 of 20 patients showed a greater than 20% reduction in pulmonary vascular resistance. ( PubMed 18978135) EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 10
Why this matters This later result is more modest than the dramatic 2003 pilot.
Therefore Navaro should not summarize the pulmonary literature as:
“VIP reliably produces major pulmonary improvement.”
Better wording Inhaled VIP has demonstrated acute pulmonary vasodilatory activity in humans, with larger clinical signals in a very small early pilot and more modest acute effects in subsequent pulmonaryhypertension research.
Human Sarcoidosis Evidence
A 2010 open-label Phase II study treated:
20 patients with histologically confirmed active sarcoidosis using nebulized VIP for four weeks. ( PubMed 20442436) The regimen was:
50 mcg four times daily for:
28 days.
The investigators reported:
• reduced spontaneous TNF-alpha production by bronchoalveolar cells
• increased regulatory T-cell activity
• altered macrophage activation markers
• improvement in cough or dyspnea reported by many participants
• no major severe safety signal
What this supports This is real human evidence that inhaled VIP can alter pulmonary immune-cell behavior .
What it does not support It does not establish that VIP:
• treats autoimmune disease broadly
• “resets immune tolerance”
• treats CIRS
• should be injected SC
• should be used indefinitely
• prevents inflammatory disease
Evidence classification Small open-label route-specific human immunologic study EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 11
VIP Is Not Simply an “Immune Tolerance Peptide”
Some current community material claims that the name:
“vasoactive intestinal peptide” is misleading and that VIP is “really” an immune-tolerance orchestrator rather than primarily vascular or intestinal. ( PeptideFox) That framing is too strong.
VIP genuinely has important immunomodulatory effects.
But it also genuinely:
• dilates blood vessels
• alters pulmonary vascular tone
• relaxes smooth muscle
• stimulates intestinal secretion
• affects cardiac physiology
• acts in the nervous system
• contributes to circadian signaling
Its original name does not describe an irrelevant side effect.
Correct Navaro wording VIP is a pleiotropic neuropeptide with important vascular , gastrointestinal, pulmonary, neuronal, circadian and immune functions. No single modern function should be presented as its one “real” biological purpose.
COVID-19 Evidence
This is one of the most important sections because online summaries often cherry-pick one trial.
2022 Randomized IV Aviptadil Trial
A multicenter randomized trial enrolled:
196 patients with critical COVID-19 respiratory failure.
Participants were randomized approximately:
2:1 to aviptadil or placebo.
Aviptadil was administered over three days using escalating 12-hour IV infusions:
• 50 pmol/kg/hour
• 100 pmol/kg/hour
• 150 pmol/kg/hour ( ClinicalTrials.gov NCT04311697)
Primary endpoint Alive and free from respiratory failure at day 60 The prespecified primary analysis did not reach statistical significance .
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 12
Secondary findings The authors reported favorable signals involving:
• 60-day survival
• respiratory-distress measures
• IL-6 changes
including a reported survival advantage in secondary analysis. ( PubMed 36044317) Correct interpretation Mixed randomized human evidence not:
definitive positive Phase 3 proof The study produced encouraging secondary signals, but the primary endpoint was not met.
2023 NIH TESICO Trial
The later NIH-supported TESICO trial provides a stronger check against overinterpreting the earlier study.
The modified intention-to-treat aviptadil comparison included:
461 participants with:
• 231 receiving aviptadil
• 230 receiving placebo
By day 90:
38% of aviptadil participants had died versus:
36% of placebo participants.
Hazard ratio:
1.04 95% CI:
0.77-1.41 p:
0.78.
The investigators concluded that IV aviptadil:
did not significantly improve clinical outcomes through day 90. (PubMed 37348524) Safety outcome The prespecified early composite safety outcome occurred in:
63% of aviptadil participants versus:
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 13
56% of placebo.
The difference was not statistically significant:
OR 1.40, 95% CI 0.94-2.08, p=0.10.
Production conclusion The strongest large randomized IV evidence is:
negative for overall clinical efficacy
Was the Negative IV Trial Just a “Route Failure”?
Some community pages now say the TESICO failure should be dismissed because VIP’s half-life is too short and IV delivery was the wrong route. ( PeptideFox) That is an interpretation, not an established finding.
In fact, the IV protocol used long continuous infusions , specifically because VIP is rapidly cleared.
The trial did not simply give one IV bolus and allow it to disappear .
Correct Navaro wording The negative TESICO result may prompt legitimate research questions about route, pulmonary targeting and timing, but the trial cannot simply be reclassified as a “delivery failure.” It was a properly controlled IV treatment study using prolonged infusions designed around VIP pharmacology.
2025 Randomized Inhaled Aviptadil Study
A newer multicenter randomized double-blind placebo-controlled study evaluated inhaled aviptadil in:
80 hospitalized COVID-19 patients across nine centers. ( PubMed 39870064) Average time to discharge was:
7.8 ± 4.0 days with aviptadil versus:
10 ± 5.0 days with placebo.
p:
0.049 The investigators also reported:
• better day-7 dyspnea scores
• greater improvement in CT lung-damage score at day 28
• no treatment dropout because of adverse effects
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 14
Mortality The reported death rate was:
5.1% with aviptadil versus:
12.2% with placebo.
The study was not large enough for that difference alone to establish a definitive mortality benefit.
Correct evidence classification Positive randomized inhaled human signal in a modest-sized COVID-19 study What it does not prove It does not prove that:
• inhaled VIP universally works for ARDS
• inhalation is superior to IV
• nasal spray is equivalent to nebulization
• SC VIP works
• VIP treats chronic pulmonary disease broadly
Inhaled vs IV
The human literature now supports a more nuanced conclusion.
IV Large randomized TESICO:
negative overall Inhaled Smaller randomized trial:
positive recovery/radiographic signal But These trials involved:
• different populations
• different disease stages
• different formulations
• different routes
• different trial sizes
• different endpoints
• different study periods
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 15
Verdict Inhaled administration has encouraging route-specific human evidence, but no direct head-to-head trial establishes inhaled or intranasal VIP as clinically superior to IV aviptadil.
Intranasal VIP / CIRS
This is the source of much of the current nasal-spray community protocol.
A 2013 study enrolled:
20 patients described as having refractory chronic inflammatory response syndrome associated with water-damaged buildings.
It was:
open-label with no randomized placebo group. ( 2013 CIRS study PDF ) Participants self-administered:
50 mcg VIP by nasal aerosol:
four times daily initially.
Treatment continued for at least:
18 months with later individual titration down and back up according to symptoms.
What the CIRS Study Reported
The investigators reported improvements involving:
• symptom counts
• quality of life
• C4a
• TGF-beta1
• MMP-9
• VEGF
• sex-hormone measures
• vitamin D
• exercise pulmonary artery pressure
• regulatory T cells
Important limitations The study had:
• only 20 participants
• no placebo group
• no blinding
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 16
• a highly selected refractory clinic cohort
• multiple prior sequential treatments
• historical/control comparisons
• extensive protocol-specific diagnostic assumptions
• investigators directly involved in the treatment model
Correct evidence classification Small uncontrolled clinic-derived intranasal human study not:
randomized Phase 2 proof of intranasal VIP and not:
validation of CIRS as an indication for VIP generally
Human Research
VIP duration has varied tremendously by indication.
Examples include:
• one inhaled dose
• four weeks of inhaled therapy
• approximately 12 weeks in pulmonary-hypertension research
• three days of IV ICU treatment
• ten days of inhaled COVID treatment
• 18 months or longer in the small CIRS nasal study
This itself demonstrates that there is no universal VIP “cycle.”
CIRS Is Not the Same Evidence Category as Sarcoidosis
This distinction is especially important online.
Sarcoidosis
• recognized inflammatory disease
• histologically confirmed population
• 20-patient open Phase II inhaled study
• bronchoalveolar immune endpoints
CIRS-WDB study
• 20-patient specialized clinic cohort
• open label
• intranasal route
• long sequential treatment program
• no placebo group
• broader disputed diagnostic framework

These should not be presented as equal-strength Phase II evidence.
Regulatory Status

Not FDA Approved for Orphan Indication . ( FDA orphan database ) An orphan designation is not drug approval.
FDA 503A Status
As of FDA’s:

FDA Substance Identity Record
FDA has a validated aviptadil substance identity record with:
UNII A67JUW790C and the human VIP sequence.
FDA explicitly states:
UNII availability does not imply regulatory review or approval. (FDA substance record) This is useful for:
molecular identity not:
approval status
FDA Orphan Designation
FDA has granted aviptadil orphan-drug designation for certain investigational indications, including sarcoidosis.
The FDA database explicitly distinguishes:
designation from:
approval. (FDA orphan database ) Production rule Do not write:
“FDA-approved orphan drug.”
unless an actual approval exists.
European Orphan Designation
EMA records a European orphan designation for aviptadil in sarcoidosis.
EMA likewise explicitly explains that orphan designation is:
not a marketing authorization and that quality, safety and efficacy must still be demonstrated before approval. ( EMA orphan designation) EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 32
Invicorp Regional Approval
Separate from standalone aviptadil development, aviptadil is used in the authorized combination:
aviptadil + phentolamine for erectile dysfunction in some European jurisdictions.
The product uses intracavernosal administration. ( BNF) Final regulatory wording U.S. standalone VIP / aviptadil: Not FDA approved FDA 503A VIP bulk status: Category 1 under evaluation Aviptadil orphan designations: Developmental designations, not approval European aviptadil + phentolamine: Authorized intracavernosal ED combination in certain jurisdictions
Important Research Note
VIP is a compound where the same exact peptide has been studied across many routes.
That makes evidence mixing especially tempting.
The page must keep at least six evidence categories separate.
1. Human IV VIP / Aviptadil
Real pharmacology and randomized clinical evidence.
Includes a large negative COVID trial.
2. Human Inhaled / Nebulized VIP
Real pulmonary human evidence.
Includes:
• pulmonary hypertension
• sarcoidosis
• COVID pneumonia
3. Intranasal VIP
Small clinic-derived human evidence exists.
The CIRS literature is not equivalent to the stronger pulmonary randomized evidence.

A dose proven or studied by one route should not be copied into another route simply because the peptide identity is the same.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 41
Critical SC Protocol Warning The widely published 50-200 mcg SC protocols are community-derived. Human SC dose-finding, efficacy and pharmacokinetic studies have not established an optimal wellness or immune protocol.
Critical COVID Warning The VIP COVID literature is mixed. The earlier 196-patient IV trial missed its primary endpoint but reported favorable secondary signals. The larger NIH TESICO trial did not show significant overall clinical benefit. A smaller later inhaled randomized study reported positive recovery and lungimaging signals. These studies should not be collapsed into either “VIP worked” or “VIP failed.”
Critical CIRS Warning The best-known intranasal CIRS protocol comes from a 20-person open-label clinic study, not a randomized placebo-controlled trial. Its 50 mcg four-times-daily regimen should remain clinic-derived research evidence rather than a universal VIP protocol.
Final Bottom Line
VIP / aviptadil has substantially more legitimate human research than many compounds sold through peptide communities.
The molecule itself is not mysterious.
VIP is a genuine endogenous:
28-amino-acid human neuropeptide and aviptadil is its synthetic pharmaceutical form.
The complication is not primarily molecular identity.
It is:
route.
Human researchers have given VIP:
• intravenously
• by nebulized inhalation
• by nasal aerosol
• directly into erectile tissue
and each route produces a different clinical exposure.
The human pulmonary evidence is genuine.
In a tiny 2003 pulmonary-hypertension pilot, inhaled VIP produced striking hemodynamic and exercise signals.
A later 20-patient study confirmed acute selective pulmonary vasodilation, but the effect was more modest.
A 20-patient sarcoidosis study demonstrated real pulmonary immune-cell changes after four weeks of inhaled VIP .
Those findings establish that VIP is biologically active in humans.
They do not establish a generalized SC peptide protocol.
The COVID program is particularly instructive.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 42
The first randomized IV trial in 196 critically ill patients missed its primary endpoint but reported favorable secondary findings.
Then the larger NIH TESICO trial provided a much harder test.
Among 461 treated participants, mortality through day 90 was approximately:
38% with aviptadil versus:
36% with placebo and investigators concluded that IV aviptadil did not significantly improve clinical outcomes.
That negative result cannot simply be discarded as:
“IV was the wrong route.”
The protocol used prolonged 12-hour infusions precisely because VIP is so rapidly cleared.
Route remains a legitimate research question, but it does not invalidate the trial.
At the same time, the later inhaled randomized study should not be ignored.
In 80 hospitalized patients, inhaled aviptadil was associated with shorter average time to discharge and favorable day-7 dyspnea and day-28 CT findings.
That means the correct conclusion is:
route-specific evidence is mixed and still developing rather than:
VIP works or:
VIP does not work.
The intranasal CIRS literature needs even more restraint.
The famous protocol really does exist.
Twenty patients received:
50 mcg intranasal VIP four times daily and were followed for at least 18 months.
The investigators reported extensive symptom and laboratory improvements.
But the study was open-label, uncontrolled, highly selected and embedded in a much larger clinic-specific sequential treatment framework.
Therefore it is legitimate to document:
50 mcg QID intranasal as the Shoemaker-study protocol but not legitimate to convert that into:
the clinically proven VIP protocol.
The lipase issue is another good example of community claims getting stronger as they are repeated.
The original study monitored lipase.
One patient developed a transient elevation without abdominal pain.
It resolved without stopping VIP .
There was no overall cohort increase.
That does not support presenting pancreatitis as VIP’s defining established toxicity.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 43
The clearest systemic pharmacology is actually much older .
Classic human IV research found VIP disappears from plasma extraordinarily quickly, with an average disappearance half-time of around:
one minute.
That number is real.
But it still does not provide:
a human SC half-life.
SC absorption can alter concentration-time behavior , and no adequate human SC PK program was identified.
Therefore the familiar community argument:
“VIP lasts one minute, so inject it twice daily” does not logically follow.
The same problem applies to current SC dosing.
PeptideFox and Peptide Dosing Protocols now publish approximately:
50-200 mcg SC with once-daily, twice-daily and five-times-weekly patterns.
Those figures should remain on Navaro because they document how VIP is actually used in peptide communities.
But they should appear under:
Community Protocols not:
Human Research Dose.
The clean hierarchy is therefore:
Human IV VIP: real evidence Human inhaled VIP: real evidence Intranasal VIP: small uncontrolled human evidence Intracavernosal aviptadil + phentolamine: legitimate regional product evidence Human SC VIP: no validated dosing framework Community SC VIP: approximately 50-200 mcg per administration, highly protocol dependent The regulatory page needs similar precision.
VIP is not an FDA-approved standalone drug.
But FDA’s May 14, 2026 bulk-substance document places:
Vasoactive Intestinal Peptide in:
503A Category 1, under evaluation.
That is important current information.
It does not constitute approval.
Aviptadil has also received orphan-drug designations, but FDA’s own database distinguishes orphan designation from drug approval.
And internationally, aviptadil is not merely hypothetical.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 44
A combination of:
25 mcg aviptadil + 2 mg phentolamine is used as an authorized intracavernosal erectile-dysfunction medicine in parts of Europe.
That is genuine pharmaceutical human use.
It is also a perfect example of why Navaro must keep route and formulation attached to every dose.
The strongest final production principle for VIP is therefore:
Same peptide does not mean same protocol.
VIP given for twelve hours through an IV infusion pump, VIP delivered directly to the lungs through a nebulizer , VIP sprayed into the nose, VIP combined with phentolamine and injected intracavernosally, and a research vial injected subcutaneously are not five interchangeable ways of doing the same treatment .
The evidence hierarchy must preserve those distinctions.
The final Navaro conclusion should therefore be:
VIP / aviptadil is a biologically well-characterized human neuropeptide with meaningful routespecific clinical research. Its strongest evidence comes from controlled or direct human IV and inhaled studies, not from community SC protocols. Intranasal CIRS evidence exists but is small and uncontrolled. A general 50-200 mcg SC wellness or immune protocol has not been clinically validated, and the peptide’s very short IV plasma half-life should not be used to manufacture an SC frequency without human SC pharmacokinetic data.

Current Community SC Protocol-Page Cluster
Current PeptideFox material describes approximately:
50-200 mcg SC with morning-timed protocols.
Current Peptide Dosing Protocols describes:
• 50 mcg once daily
• 100 mcg one to two times daily
• 200 mcg one to two times daily
• 200 mcg five times weekly
depending on the protocol tier . Both sites acknowledge that these are not FDA-approved SC dosing schedules.
(PeptideFox) Current evidence classification Community protocol-page range: approximately 50-200 mcg per SC administration with commonly discussed frequencies ranging from:
• once daily
• twice daily
• five days weekly
Clinically validated SC dose None
PeptideDosages and Red Fox
I did not verify a current dedicated VIP protocol page from PeptideDosages or Red Fox during this final live search.
The older Navaro audit likewise had no exact indexed PeptideDosages protocol and only a Red Fox VIP product/protocol-related listing rather than a clear clinical protocol.
Production rule Do not invent a four-source “consensus” where a current exact protocol cannot be verified.

Human IV Dose Anchors
Classic Human Physiology
VIP has been administered by continuous IV infusion in healthy humans at graded pmol/kg/min infusion rates.
The classic 1978 pharmacokinetic study involved only four healthy subjects and found extraordinarily rapid disappearance after infusion stopped. ( PMC classic PK )
COVID-19 Phase 2b/3 Program
The 196-patient randomized study used three successive 12-hour infusions:
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 7
Day 1 50 pmol/kg/hour Day 2 100 pmol/kg/hour Day 3 150 pmol/kg/hour These were ICU-style continuous infusion protocols. ( ClinicalTrials.gov NCT04311697) Critical warning This does not support converting:
50 pmol/kg/hour IV into:
50 mcg SC or any other route.
European Authorized Product Context
Invicorp
Aviptadil is part of an authorized intracavernosal erectile-dysfunction combination in some European jurisdictions.
The product combines:
25 mcg aviptadil with:
2 mg phentolamine per intracavernosal administration.
The BNF lists a maximum frequency of:
once daily and:
three times weekly. (BNF) Why this matters This is genuine clinical/product dosing.
What it does not establish It does not validate:
• standalone VIP injection
• SC VIP
• nasal VIP
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 8
• IV wellness use
• immune protocols
• CIRS protocols
because:
intracavernosal aviptadil + phentolamine ≠ standalone systemic VIP
Expected Results Timeline
The timeline must stay route-specific.
Time period Human clinical/research evidence Community / clinic-derived context Minutes IV and inhaled vascular effects can occur rapidly because VIP is a fast-acting signaling peptide.
Flushing or lightheadedness may also appear quickly.
Acute inhalation Pulmonary-hemodynamic studies measured changes soon after a 100 mcg inhaled dose.
Not evidence for SC wellness effects.
Days 1-10 COVID inhalation studies evaluated early respiratory recovery during active treatment.
Community SC users may report early subjective effects, but no controlled SC timeline exists.
4 weeks Sarcoidosis research found pulmonary immune-cell changes after four weeks of inhaled therapy.
CIRS/community material often discusses symptom changes over weeks.
12 weeks The 2003 pulmonary-hypertension pilot showed hemodynamic and exercise changes by 12 weeks.
Does not establish a universal 12-week VIP cycle.
Months Small pulmonary and CIRS studies followed patients longer .
Long-term intranasal CIRS reports are uncontrolled.
Human SC timeline Not established Community only
The CIRS Dose Must Not Become a Universal Nasal Dose
The study used:
50 mcg per nasal dose initially:
four times daily but this was one specific clinic protocol.
It does not establish that:
• 50 mcg is optimal
• four sprays per day is optimal
• 8 sprays is better
• nasal VIP should be used indefinitely
• the same protocol applies to other inflammatory conditions
Verdict Human clinic-derived intranasal protocol with:
very low certainty of generalizable efficacy
Does Twice-Daily SC Dosing Make Sense?
Community sites commonly use:
100 mcg twice daily or similar schedules.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 19
The reasoning is largely based on:
• VIP’s short plasma persistence
• repeated receptor exposure
• community experience
No comparative human SC study establishes that:
BID > once daily or:
BID > 5× weekly Verdict Community-derived frequency not:
clinically optimized schedule
Does Morning Dosing Matter?
PeptideFox describes morning-timed VIP protocols linked to circadian biology. ( PeptideFox) VIP genuinely participates in circadian signaling in the suprachiasmatic nucleus.
But this does not establish that an exogenous SC dose:
must be administered in the morning to produce clinical benefit.
Verdict Circadian mechanistic rationale exists Human SC timing superiority: Not established
Fasting / Food Requirement
There is no established human requirement that injected or nasal VIP be administered:
• fasted
• fed
• before meals
• after meals
Verdict No evidence-based food requirement EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 20
Exercise Timing
There is no controlled evidence showing VIP should be used:
• before workouts
• after workouts
• only on active days
Verdict No evidence-based exercise-timing protocol
Duration / Cycle
Community SC Cycle
Community sources often describe continuous use rather than traditional peptide cycles.
Some also discuss:
• 4 weeks
• 8 weeks
• 12 weeks
• ongoing maintenance
Human SC cycle Not established
Receptor Reset
No evidence VIP works mainly through VPAC receptors, but there is no validated clinical:
• receptor-reset cycle
• weekend break
• 5-on / 2-off requirement
• washout period
• “resensitization” schedule
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 21
Verdict No evidence-based receptor-reset requirement
What Does VIP Actually Do?
VIP is a pleiotropic signaling peptide rather than a one-pathway drug.
VPAC1 and VPAC2
The principal VIP receptors are:
VPAC1 and:
VPAC2 These are class B G-protein-coupled receptors.
Both can activate:
adenylyl cyclase → cAMP with downstream signaling varying by cell type.
VIP receptor expression differs among:
• blood vessels
• airway tissue
• gastrointestinal tissue
• immune cells
• central nervous system
• endocrine tissues
Simple version VIP does not have one “immune receptor .”
It sends similar molecular signals into very different tissues, producing different effects depending on where the receptor is located.
PAC1 Is Not an Equal VIP Receptor
PAC1 is primarily a receptor for:
PACAP not VIP .
VIP can interact with PAC-family signaling under some circumstances, but Navaro should not list:
VPAC1 / VPAC2 / PAC1 as three equivalent primary VIP receptors.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 22
Production wording Primary VIP receptors: VPAC1 and VPAC2
Vascular Effects
VIP is a potent vasodilatory peptide.
Classic human infusion studies demonstrated effects on:
• vascular tone
• blood pressure
• heart rate
• circulation
Its vascular activity is not merely an obsolete naming artifact.
Clinical relevance This is why systemic exposure can produce:
• flushing
• reduced blood pressure
• reflex increases in heart rate
• lightheadedness
Pulmonary Effects
VIP can influence:
• pulmonary vascular resistance
• airway smooth muscle
• epithelial signaling
• surfactant-related biology
• pulmonary immune cells
Human pulmonary-hypertension research confirms that at least some pulmonary vascular effects occur in living humans. ( JCI study)
Gastrointestinal Effects
VIP stimulates intestinal fluid and electrolyte secretion.
Excess endogenous VIP is classically associated with severe watery diarrhea.
This physiology is consistent with diarrhea observed during systemic VIP administration.
Why this matters Calling VIP “not really intestinal” is biologically misleading.
Its gastrointestinal actions are substantial.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 23
Immune Effects
VIP can influence:
• macrophages
• dendritic cells
• T cells
• regulatory T-cell biology
• inflammatory cytokine production
The sarcoidosis study provides direct human evidence of pulmonary immune-cell changes.
Much broader claims about:
• autoimmune disease treatment
• infection tolerance
• “immune reset”
are still largely extrapolated from preclinical immunology. ( PubMed 20442436)
Circadian Biology
VIP plays an important role in neuronal communication within the suprachiasmatic nucleus and circadiannetwork synchronization.
That supports a mechanistic relationship with circadian biology.
But It does not establish an exogenous:
“morning VIP optimization protocol” for humans.
Community Protocol Comparison
Source Current protocol Correct classification PeptideFox ~50-100 mcg intranasal; ~50-200 mcg SC, including morning-timed discussion Community / secondary Peptide Dosing Protocols 50 mcg daily to 200 mcg 1-2× daily SC; separate Shoemaker-style nasal protocol Community / clinic-derived PeptideDosages No exact current VIP page verified in this pass Not confirmed Red Fox Peptides No exact current dedicated VIP protocol verified in this pass; older Navaro audit found product/protocol-related coverage Community only / not currently verified
Community Claim Audit
“VIP’s real function is immune tolerance”
Too strong VIP has meaningful immune effects but also well-established vascular , gastrointestinal, pulmonary and neuronal physiology.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 33
“VIP has a one-minute half-life”
Substantially supported for classic human IV plasma disappearance but should be written as:
approximately one-minute IV plasma disappearance half-time rather than universal route-independent half-life.
“Inhaled/nasal VIP outperforms IV”
Not established Different trials suggest different outcomes, but no head-to-head study proves route superiority.
“The large IV trial failed only because the route was wrong”
Speculative interpretation TESICO used prolonged continuous infusion specifically to provide systemic exposure.
“50-200 mcg SC is a research dose”
Community dose No completed human SC dose-finding study established this range.
“100 mcg BID is the standard protocol”
No It is one of the more commonly published community schedules.
“Morning dosing is clinically superior”
No evidence Circadian biology provides mechanistic rationale, not clinical timing validation.
“VIP requires continuous use”
Not established for SC wellness use Human studies range from a single dose to months.

Question Verdict Does that combination establish SC VIP dosing? No
Contraindications
Lipase and the “Pancreatitis Risk” Claim
This deserves a dedicated correction.
Current community protocol pages sometimes state that:
pancreatitis is VIP’s main documented safety risk and that lipase monitoring is therefore universally mandatory.
The original 2013 CIRS study did monitor lipase.
But what it actually reported was:
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 17
• lipase did not rise across the cohort
• one patient had a transient elevated lipase
• the patient had no abdominal pain
• the elevation resolved without stopping VIP
What this supports Lipase was part of the specific CIRS clinic-monitoring protocol.
What it does not establish It does not establish that:
• pancreatitis is VIP’s principal clinical toxicity
• VIP commonly causes pancreatitis
• lipase monitoring is a universal evidence-based requirement across all routes
• an isolated elevated lipase is equivalent to pancreatitis
Correct Navaro wording Lipase monitoring has been incorporated into some intranasal CIRS protocols. The small 2013 study reported one transient asymptomatic lipase elevation without an overall cohort increase. This does not establish pancreatitis as a common or defining VIP adverse effect.
Human Pharmacokinetics
Plasma Half-Life
VIP is genuinely very short-lived in human plasma.
The classic 1978 healthy-volunteer IV study reported first-order disappearance after the infusion ended, with an average disappearance half-time of approximately:
1 minute. ( PMC classic PK ) This is one of the rare community half-life claims that has a real human foundation.
However , the proper wording is:
Human IV plasma disappearance half-time: approximately one minute in a classic small infusion study not simply:
“VIP half-life = one minute under every route.”
Why Route Still Matters
A plasma half-life measured after IV infusion does not automatically tell us:
• absorption from nasal mucosa
• SC absorption rate
• tissue residence
• pulmonary deposition
• local receptor exposure
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 18
• duration of downstream signaling
Therefore the one-minute IV plasma half-life should not be turned into:
“inject every hour” or:
“twice daily SC is scientifically required.”
Human SC Half-Life
Not established I did not identify a dedicated human SC PK study of ordinary unmodified VIP establishing:
• absorption half-life
• Tmax
• Cmax
• bioavailability
• elimination profile
Verdict Do not use the IV one-minute number as an SC dosing interval.
Does SC VIP Make Pharmacokinetic Sense?
It is possible that SC absorption could create a different exposure curve than IV dosing.
But that must be measured.
There is currently no controlled human SC PK program showing that:
• 50 mcg
• 100 mcg
• 200 mcg
produces therapeutically meaningful systemic exposure.
Correct classification Pharmacologically plausible route but:
clinically inadequately characterized
Safety at a Glance
Human IV / Systemic Effects
Known physiologic and clinical concerns include:
• hypotension
• flushing
• tachycardia
• diarrhea
• fluid/electrolyte consequences if diarrhea is severe
These are consistent with VIP’s established human vascular and intestinal pharmacology.
Hypotension
Systemic VIP can lower blood pressure.
This has been observed in:
• classic healthy-volunteer infusion research
• ARDS programs
• ICU aviptadil programs
Classification Established pharmacologic effect The degree depends heavily on:
• infusion rate
• clinical status
• route
• concurrent medications
Tachycardia
Heart-rate increases can occur secondary to systemic vasodilation and hemodynamic changes.
Classification Known systemic pharmacologic effect
Diarrhea
This is a particularly biologically coherent adverse effect.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 25
VIP stimulates intestinal secretion, and higher systemic exposure can produce watery diarrhea.
The NIH TESICO protocol specifically identified:
hypotension and:
diarrhea as the primary treatment-intolerance concerns during aviptadil infusion.
Flushing
Facial or generalized flushing can occur because of cutaneous vasodilation.
Classification Known pharmacologic effect
Intranasal Safety
The small CIRS study reported overall good tolerability during long-term use, but a:
20-person uncontrolled study cannot establish the incidence of uncommon or serious adverse effects.
Possible route-specific effects can include:
• local nasal irritation
• congestion
• transient systemic vasodilatory symptoms
The actual incidence is not well defined.
SC Safety
Not adequately characterized Community reports and mechanistic expectations may include:
• flushing
• dizziness
• injection-site effects
• diarrhea
• headache
but there is no developed human SC adverse-event database allowing reliable incidence estimates.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 26
Blood Pressure and Other Vasodilators
Because VIP is a potent vasodilator , additional caution is biologically reasonable in people with:
• low baseline blood pressure
• medications that substantially lower blood pressure
• other strong vasodilators
Evidence classification Mechanism-based caution Specific interactions and magnitudes are product and route dependent.
Severe Diarrheal Illness
The TESICO protocol excluded severe diarrhea because VIP itself can worsen intestinal secretion.
That is a trial-specific safety decision consistent with known physiology.
Production wording Existing significant diarrhea is a practical systemic-use caution not:
an FDA-labeled universal contraindication
Pregnancy / Breastfeeding
No general standalone VIP safety program establishes safety for elective systemic or wellness use during:
• pregnancy
• breastfeeding
Classification Safety insufficiently established
Children / Adolescents
There is no established general pediatric wellness or SC protocol.
Classification Dose and safety not established EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 27
Cancer / Tumor Biology
VIP receptors are expressed in multiple tissues, and experimental VIP biology can influence:
• cell signaling
• angiogenesis
• immune regulation
• growth-related pathways
That creates mechanistic questions in active malignancy.
However , there is no human clinical evidence showing that ordinary exogenous VIP:
causes cancer or:
accelerates all tumors.
Correct classification Theoretical / disease-specific caution not:
formal universal contraindication
Formal Contraindications
Standalone VIP
There is no FDA-approved standalone VIP label providing a universal contraindication list.
Therefore Navaro should avoid presenting mechanistic cautions as though they were official labeling.
Invicorp Contraindications Are Product-Specific
The authorized aviptadil/phentolamine intracavernosal product has its own:
• indication
• contraindications
• injection-specific risks
• dosing restrictions
Those rules apply to:
that combination product not automatically to nasal or SC standalone VIP . ( BNF)

• gut-related goals
• inflammatory goals
• recovery

NAD+, Glutathione and Other Wellness Compounds
They may appear alongside VIP in clinic or community protocols.
Production rule Concurrent use does not establish pharmacologic synergy.
“Pancreatitis is VIP’s main documented risk”
Overstated The CIRS paper monitored lipase and reported one transient asymptomatic elevation without an overall rise.
“50 mcg intranasal four times daily is a proven CIRS treatment”
No It was the protocol used in one small uncontrolled 20-person clinic study.

The supplied chapter does not establish a separate side-effect list for this compound.
Compatibility
Same-Vial / Same-Syringe Compatibility
There is no reason to assume that because two peptides are used in the same protocol they are:
• chemically compatible
• physically compatible
• stable at the same pH
• protected from degradation
• safe to combine in one vial
Verdict Exact formulation evidence required
4. Intracavernosal Aviptadil Combination
Authorized regional drug-product evidence exists for:
aviptadil + phentolamine This evidence is route and combination specific.
5. Community SC VIP
This is where the current:
• 50 mcg
• 100 mcg
• 200 mcg
• once-daily
• twice-daily
• five-days-weekly
protocols reside.
These are not validated human SC regimens.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 36
6. Mechanistic / Preclinical VIP
Large literature involving:
• immune signaling
• pulmonary protection
• circadian biology
• neurobiology
• autoimmune models
This supports biological plausibility.
It does not establish a wellness protocol.

Storage
Formulation / Stability
VIP presents a particularly important stability problem because it is a relatively fragile, rapidly degraded 28-amino-acid peptide.
There is no universal FDA-approved standalone injectable VIP product giving one validated:
• vial concentration
• diluent
• pH
• reconstitution method
• refrigerated lifetime
• 14-day rule
• 28-day rule
Production rule Do not write “all VIP lasts 28 days after reconstitution.”
Stability must be tied to the exact formulation.
EDUCATIONAL RESEARCH SUMMARY | VIP / AVIPTADIL | AUDITED EVIDENCE STANDARD Page 30
Nasal Spray Is Not the Same as a Reconstituted SC Vial
A pharmacy-prepared nasal formulation can differ in:
• concentration
• excipients
• pH
• sterility requirements
• container
• spray volume
• preservative system
Therefore:
50 mcg nasal aerosol cannot be converted into:
50 mcg SC merely because the numerical mass is identical.
