Dosing evidence

The pulse pattern matters as much as the dose

Natural GnRH is not released continuously.

It is released in brief pulses.

That timing pattern is fundamental to normal reproductive endocrine function.

Appropriate intermittent GnRH stimulation promotes pituitary:

LH + FSH secretion while prolonged or sustained receptor stimulation can eventually lead to:

GnRH-receptor desensitization and reduced gonadotropin output.

Simple version Gonadorelin is not a compound where:

more frequent = more effective.

The pituitary responds to the pattern of GnRH exposure.

That means a schedule such as:

100-200 mcg three times per day is not automatically a good approximation of normal physiology merely because it is “frequent.”

And:

50-150 mcg Monday / Wednesday / Friday is not meaningfully “pulsatile” in the physiologic sense.

The historical medical concept of pulsatile GnRH involved small automated doses repeated approximately every 60-120 minutes, not several isolated injections per week.

Central production sentence Gonadorelin has strong human evidence when used to reproduce missing hypothalamic GnRH signaling through genuinely pulsatile administration. Modern intermittent SC bolus protocols are different exposures with a much weaker evidence base.

Dose at a Glance

FDA-Approved Current Dose

There is no currently marketed FDA-approved U.S. gonadorelin product providing a modern general-use protocol.

But historical FDA-approved gonadorelin products existed.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 9

That distinction matters.

Historical Pulsatile Human Therapy

Historical human therapeutic anchor:

5 mcg every 90 minutes SC or IV by programmable pulsatile pump Clinical individualized range:

Approximately 1-20 mcg per pulse , depending on indication and endocrine response.

Production rule Do not compress this into:

“Typical gonadorelin dose = 100 mcg.”

There is no one dose without defining the purpose and exposure pattern.

Diagnostic Gonadorelin

Gonadorelin has also been used as a GnRH stimulation test.

A single approximately:

100 mcg IV or SC bolus has legitimate human diagnostic precedent for assessing pituitary LH/FSH response.

Critical distinction A single diagnostic:

100 mcg bolus does not validate:

100 mcg repeatedly during TRT.

Current Community Protocol Comparison

The current community market is internally inconsistent.

Current pages span:

• a few injections weekly

• daily injections

• multiple daily injections

• escalating schedules

The sources are not converging on the historic clinical physiology.

The Current “Pulsed Protocol” Is Not Actually Pulsatile

Calling the M/W/F schedule:

Pulsed Protocol is misleading.

Technically every injection produces a pulse.

But in reproductive endocrinology, pulsatile GnRH therapy means repeated short-interval administration designed to replicate physiologic hypothalamic secretion.

Monday/Wednesday/Friday dosing does not do that.

Rename Community Intermittent Bolus Protocol

Does Three Times Daily Mimic Natural Pulses?

Poorly True pulsatile GnRH therapy historically uses small doses approximately every 60-120 minutes.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 11

A 90-minute pulse interval produces approximately:

16 pulses/day.

A TID protocol produces:

3 exposures/day.

Better wording Several manual injections per day may create intermittent GnRH exposure but should not be described as faithfully reproducing physiologic pulsatility.

Does 2-3 Times Weekly Make Sense?

It may generate short transient LH/FSH responses.

But there is no strong evidence that:

50-200 mcg 2-3× weekly maintains continuous fertility-related gonadotropin support.

Given gonadorelin’s brief action, large gaps between doses are biologically very different from pulsatile replacement.

Verdict Community/TRT practice not:

physiologic pulsatile replacement

Once-Daily Dosing

A dose can induce a temporary LH/FSH response.

That does not mean once-daily dosing reproduces normal HPG-axis signaling across the day.

Verdict Intermittent pharmacologic stimulation not:

physiologic pulse replacement

Continuous Gonadorelin

Sustained GnRH receptor activation can lead to desensitization.

But native GnRH and long-acting GnRH agonists do not have identical pharmacokinetics.

Better wording Continuous or sustained GnRH-receptor stimulation can desensitize pituitary gonadotrophs and suppress LH/FSH. This is a fundamental reason pulsatile and continuous GnRH signaling have opposite endocrine consequences. The exact desensitization risk depends on dose, exposure pattern and formulation.

Do not write “Any incorrectly timed dose causes chemical castration.”

That goes beyond the evidence.

Receptor Reset

There is no evidence-based community:

• 4-week reset

• 8-week reset

• weekend washout

• receptor recovery cycle

Verdict No standard receptor-reset cycle The real biological issue is:

pulse pattern not:

peptide-cycle mythology.

Timing at a Glance

Morning required No Night required No Fasting required No Food requirement No established relevance Workout timing No established relevance What matters clinically is pulse frequency and endocrine context , not morning versus bedtime.

“Gonadorelin half-life = 2 minutes.”

The broader human PK is biphasic.

Subcutaneous Pharmacology

SC administration has demonstrated bioavailability and pituitary LH/FSH response in humans.

Historical relative SC/IV bioavailability estimates varied by dose and study conditions.

Important That does not establish pharmacokinetic equivalence between:

SC pump micro-pulses and:

large intermittent community boluses.

Renal Function

The kidney is a major route of gonadorelin metabolism and clearance.

Historical human data indicate renal failure can prolong gonadorelin half-life and reduce clearance.

Production classification Established pharmacokinetic consideration

Cycle Length

There is no single Gonadorelin cycle.

months to years depending on the goal, particularly when puberty or spermatogenesis is being induced.

Community TRT adjunct

No validated cycle Therefore A generic:

Expected Results Timeline

Acute Human Response

LH and FSH can rise relatively quickly after a gonadorelin stimulation dose.

That is why gonadorelin has historically been useful diagnostically.

This supports:

acute pituitary responsiveness not:

long-term fertility restoration after one injection.

Female Pulsatile Therapy

Clinical outcomes are measured across treatment cycles, commonly over several weeks.

Male Hypogonadotropic Hypogonadism

Testosterone may normalize earlier than full spermatogenesis.

Restoration of sperm production can take:

months because spermatogenesis itself is a slow biological process.

Important This makes a generic:

Weeks 1-4 peptide timeline misleading if presented as a universal gonadorelin treatment cycle.

Historical therapy used frequent small doses designed to reproduce hypothalamic GnRH pulses. A Monday/Wednesday/Friday injection schedule, once-daily injection or even three manual injections per day should not automatically be described as equivalent physiologic pulsatile replacement.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 28

Critical TRT Warning Evidence that pulsatile gonadorelin restores fertility in true hypothalamic GnRH deficiency does not prove that intermittent gonadorelin reliably preserves fertility, intratesticular testosterone or sperm production during suppressive testosterone therapy.

Critical hCG Warning Gonadorelin stimulates the pituitary upstream. hCG acts downstream at the LH receptor . They are not pharmacologically interchangeable.

Critical Regulatory Warning Gonadorelin has a real history of FDA-approved human use. Those approvals were tied to specific products and indications. They do not make modern compounded or research-vial Gonadorelin products FDA approved. Likewise, 503B Category 1 means under evaluation, not approved.

Protocol logic check

Research

At a Glance

What is Gonadorelin?

Gonadorelin is the synthetic form of the naturally occurring human hormone:

gonadotropin-releasing hormone, or GnRH It is a 10-amino-acid peptide released naturally by the hypothalamus.

Its sequence is:

pGlu-His-Trp-Ser-Tyr-Gly-Leu-Arg-Pro-Gly-NH2 Its main target is the:

GnRH receptor , GnRHR, on pituitary gonadotroph cells Activation of that receptor causes release of:

• luteinizing hormone, LH

• follicle-stimulating hormone, FSH

Those hormones then regulate ovarian and testicular function, including:

• ovulation

• estrogen production

• testosterone production

• spermatogenesis

• fertility

Gonadorelin therefore acts upstream of the gonads. It does not directly replace testosterone, LH, FSH or hCG.

The Most Important Gonadorelin Principle

How Well Studied Is Gonadorelin?

Very well studied biologically and historically in reproductive medicine.

A better evidence hierarchy is:

Evidence area Assessment Human GnRH physiology Very strong Pituitary stimulation/testing Established human use Pulsatile female fertility treatment Strong historical clinical evidence Pulsatile male hypogonadotropic-hypogonadism treatment Meaningful human evidence Modern pulsatile GnRH research Active / ongoing Intermittent TRT-adjunct gonadorelin Weak relative to classical pulsatile therapy 50-200 mcg intermittent SC community regimens Community-derived Long-term non-pulsatile SC optimization Not established Overall evidence rating Strong human reproductive-endocrine evidence, but much weaker evidence for modern community/TRT dosing patterns.

Historical FDA Approval

This is one of the biggest factual corrections to the current Navaro page.

FDA records show that:

gonadorelin acetate under the trade name:

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 2

Lutrepulse received U.S. marketing approval on:

October 10, 1989 for induction of ovulation in women with hypothalamic amenorrhea associated with deficient or abnormal endogenous GnRH secretion.

Therefore remove:

“No approved FDA label was identified for this entry.”

That statement is historically incorrect.

Correct wording Gonadorelin acetate historically had an FDA-approved drug product, Lutrepulse, for pulsatile ovulation induction in hypothalamic amenorrhea. That product is no longer marketed in the United States. Current compounded gonadorelin products and modern TRT protocols are not therefore automatically FDA-approved regimens.

FDA records also document historical gonadorelin drug products such as Factrel. Historical human Factrel was used diagnostically and later discontinued.

Why Historical Approval Matters

Gonadorelin should not be grouped with compounds where there is little or no established therapeutic human use.

Gonadorelin has a legitimate medical history.

But that history also makes the evidence problem more specific:

The strong evidence is for carefully timed reproductive-endocrine protocols, especially true pulsatile delivery.

It does not validate arbitrary modern peptide-vial schedules.

Pulsatile Gonadorelin

The strongest treatment evidence

Pulsatile GnRH therapy was designed to imitate the normal hypothalamic pulse generator .

Historically, treatment used an infusion pump delivering very small doses at short regular intervals.

Historical human therapeutic anchor 5 mcg every 90 minutes by SC or IV pulsatile pump Broader individualized human range Approximately:

1-20 mcg per pulse with dose individualized by indication and endocrine response.

Historical programs explored a wider range, but the central clinical model remained small microgram pulses repeated at short intervals rather than large intermittent boluses.

This is the key evidence anchor for Navaro.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 3

Modern Human Pulsatile Research

Pulsatile GnRH remains a clinically relevant concept rather than purely historical medicine.

Modern randomized Phase 3 evidence

A multicenter randomized, double-blind, placebo-controlled Phase 3 study evaluated subcutaneous pulsatile gonadorelin acetate through an OmniPod pump in women with primary amenorrhea and hypogonadotropic hypogonadism.

Studied pulse doses included:

• 10 mcg per pulse

• 15 mcg per pulse

• 20 mcg per pulse

This modern program is especially important because it continued to usesmall, frequent pump-delivered pulses, not the much larger intermittent injections used in current peptide-community protocols.

Current development

Current ClinicalTrials.gov development also includes a Phase 1 pharmacokinetic study of subcutaneous pulsatile GnRH delivered through an OmniPod system in healthy female subjects . The study supports renewed development of pulsatile GnRH for hypothalamic amenorrhea, but it should be described accurately as a PK study, not an efficacy trial in women with hypothalamic amenorrhea.

2022 Functional Hypothalamic Amenorrhea Cohort

A 25-year retrospective cohort published in 2022 evaluated:

66 women undergoing:

212 pulsatile GnRH treatment cycles for functional hypothalamic amenorrhea.

Reported outcomes included approximately:

• 96% ovulation per cycle

• 65.9% live birth per treatment

• 75% monofollicular ovulation

• 1.6% twin pregnancy rate

with no significant adverse effects reported in the cohort.

Evidence classification Meaningful modern human observational evidence supporting pulsatile GnRH therapy in appropriately selected hypothalamic amenorrhea.

Historical Female Human Evidence

The historical Lutrepulse clinical database included:

268 women receiving gonadorelin acetate or related gonadorelin formulations through programmable pulsatile pumps.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 4

Routes included:

• IV

• SC

• both IV and SC

Doses across development ranged broadly, but 90 minutes was the most common pulse interval.

Primary hypothalamic amenorrhea

Among:

48 women with primary hypothalamic amenorrhea:

45/48, or 94%, ovulated.

Among the women wishing to become pregnant:

25/43, or 58%, became pregnant.

Why this matters That is substantial direct human therapeutic evidence.

A blanket evidence rating of “Limited” does not accurately describe Gonadorelin’s established reproductive indication.

Female Results Timeline

First 10 Days

Ovulation was uncommon during the earliest treatment period.

Fewer than approximately:

5% of successful intervals resulted in ovulation before day 10.

Around Days 15-21

Approximately half of successful cycles had not ovulated until after day 15.

Almost:

90% of eventual ovulations had occurred by approximately day 21.

By Day 28

About:

95% of successful treatment intervals had resulted in ovulation by day 28.

Historical product guidance generally expected response within approximately:

2-3 weeks.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 5

Once ovulation occurred, the same pulse dose and frequency could historically be continued for approximately:

another two weeks for luteal support.

Male Congenital Hypogonadotropic Hypogonadism

Pulsatile gonadorelin also has meaningful male human evidence.

A 2019 study comparing a pulsatile gonadorelin pump with conventional gonadotropin treatment in men with congenital hypogonadotropic hypogonadism found earlier spermatogenesis and favorable testosterone restoration in the pulsatile-gonadorelin group.

Modern male CHH programs continue to use true pulse therapy.

Published regimens include approximately:

7-15 mcg per pulse every 90 minutes and other studies use approximately:

5-10 mcg every 90 minutes with adjustment according to hormonal and clinical response.

A 2025 infant CHH study used:

5 mcg every 90 minutes SC for approximately:

3-5 months with a pulsatile pump.

Evidence classification Real human therapeutic evidence but specifically in:

hypothalamic GnRH deficiency with a responsive pituitary/gonadal axis not:

general male hormone optimization.

Male CHH Timeline

Early Months

Pulsatile GnRH can restore:

• LH

• FSH

• testosterone

and begin gonadal maturation in responsive patients.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 6

Approximately 3-6 Months

Meaningful changes may emerge in:

• testosterone

• testicular development

• reproductive maturation

• early spermatogenic response

6-12+ Months

Spermatogenesis frequently requires much longer treatment.

Male fertility outcomes should therefore be assessed over months, not through a generic four-week peptide cycle.

What May Gonadorelin Help With?

Hypothalamic Amenorrhea

This is one of its best-established therapeutic uses.

When the hypothalamus fails to produce adequate GnRH pulses but the pituitary and ovaries remain capable of responding, pulsatile gonadorelin can recreate the missing upstream signal.

Evidence Strong historical human evidence

Hypogonadotropic Hypogonadism

Pulsatile GnRH may restore:

• LH

• FSH

• testosterone

• testicular growth

• spermatogenesis

when the disorder is primarily hypothalamic rather than pituitary or gonadal.

Evidence Meaningful human evidence

Male Fertility

Proper pulsatile GnRH can restore endogenous gonadotropin secretion and support spermatogenesis in selected men with congenital or hypothalamic GnRH deficiency.

That is legitimate fertility medicine.

But it should not be simplified into:

“Gonadorelin preserves fertility on TRT.”

Those are different clinical questions.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 7

The Defect Location Matters

Gonadorelin works best when the reproductive-axis defect is:

upstream at the hypothalamus while the:

pituitary and gonads remain capable of responding.

Simple version Hypothalamus missing GnRH -> gonadorelin can replace the missing signal.

Pituitary unable to respond -> gonadorelin may fail.

Gonads unable to respond -> gonadorelin may raise gonadotropins without restoring normal gonadal function.

Gonadorelin During TRT

This is where the modern evidence becomes substantially weaker .

Exogenous testosterone creates negative feedback at the:

• hypothalamus

• pituitary

which suppresses normal:

• GnRH

• LH

• FSH

and can reduce:

• intratesticular testosterone

• spermatogenesis

• fertility

A few gonadorelin injections per week do not recreate the normal GnRH pulse generator .

Production classification TRT-adjunct gonadorelin is a clinic/community practice with a physiologic rationale, but it should not be presented as equivalent to established pulsatile GnRH fertility therapy.

Why CHH Evidence Cannot Simply Be Applied to TRT

CHH and TRT suppression may both produce low LH/FSH.

But the physiology creating that state is different.

CHH Problem:

insufficient endogenous GnRH Solution:

replace GnRH in physiologic pulses EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 8

TRT Problem:

intact feedback system is intentionally suppressed by exogenous androgen Giving an occasional GnRH bolus does not remove the testosterone-mediated negative-feedback environment.

Production conclusion Successful pulsatile GnRH therapy in CHH does not establish that a few Gonadorelin injections each week will maintain the HPG axis during TRT.

Gonadorelin vs hCG

This needs its own section.

Gonadorelin

Acts at the:

pituitary GnRH receptor to stimulate endogenous:

LH + FSH if the pituitary remains responsive.

hCG Acts primarily as an:

LH-receptor agonist at the testes.

Simple pathway Gonadorelin -> pituitary -> LH/FSH -> testes versus:

hCG -> LH receptor on testes This explains why the two are not interchangeable during testosterone-induced pituitary suppression.

Production verdict Gonadorelin is not a drop-in replacement for hCG.

Regulatory Status

FDA approved historically?

Yes Gonadorelin acetate / Lutrepulse received FDA marketing approval in 1989 for hypothalamic amenorrhea.

Factrel

A separate human Gonadorelin hydrochloride product, Factrel, was historically approved for diagnostic use and later discontinued.

Do not mistake a current veterinary Factrel label for the historical human Factrel label.

Critical regulatory distinction This is:

503B not:

503A and:

FDA 2026 Warning Letter

FDA issued a 2026 warning letter concerning a seller marketing a product as:

Gonadorelin for claims involving testosterone production and spermatogenesis.

FDA treated that seller’s product as an:

unapproved new drug and emphasized risks associated with unapproved injectable products.

Critical distinction This does not mean the Gonadorelin molecule was never FDA approved.

It means:

that seller’s specific product and claimed human uses were not covered by an approved application.

Diagnostic evidence

TRT-adjunct evidence

Weak / practice-derived This should not inherit the certainty of classical pulsatile GnRH therapy.

Final Bottom Line

Gonadorelin is almost the opposite evidence problem from compounds like TB-500.

The core molecule and physiology are extremely well established.

Gonadorelin is synthetic human GnRH, and GnRH control of pituitary LH and FSH is foundational reproductive endocrinology.

Gonadorelin also has a real therapeutic history. FDA records show that Lutrepulse was approved in 1989 for induction of ovulation in hypothalamic amenorrhea.

So the old statement that no approved regimen or label was identified is wrong and should be removed.

But that does not make the current community protocol correct.

The live page has used:

100-200 mcg three times daily as its standard protocol and described that as a way to mimic natural hormonal pulses.

That is the biggest protocol problem.

True pulsatile GnRH replacement historically used much more frequent, smaller , pump-delivered pulses. A central clinical anchor is:

5 mcg every approximately 90 minutes with individualized human pulse doses commonly around:

1-20 mcg per pulse.

The body does not release GnRH three times per day.

Likewise, the live Monday/Wednesday/Friday schedule should not be called:

Pulsed Protocol.

It is:

Community Intermittent Bolus Protocol.

Current community sources make the evidence problem even clearer . Peptide protocol pages publish much larger intermittent injections, commonly around:

50-200 mcg per administration several times weekly, while other pages use daily or multiple-daily schedules.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 29

That is not a clinical consensus.

It is a collection of community schedules that have drifted far away from the best-established human Gonadorelin model.

The male fertility literature is also important.

Pulsatile GnRH can restore testosterone, testicular function and spermatogenesis in men with congenital hypogonadotropic hypogonadism when the pituitary and testes remain responsive.

Modern studies continue to use small pulses approximately every 90 minutes , further confirming that true physiologic replacement has not shifted to a Monday/Wednesday/Friday injection model.

But those data should not be transferred directly to men receiving TRT.

TRT suppresses the HPG axis through negative feedback. Intermittent Gonadorelin may produce temporary LH/FSH responses in some men, but evidence that a few injections per week reliably preserve sperm production or intratesticular testosterone is much weaker .

And this is why the hCG comparison matters.

Gonadorelin acts:

hypothalamic signal replacement -> pituitary -> LH/FSH while hCG acts:

directly at the LH receptor on the testes.

Those are not interchangeable strategies.

The desensitization section also requires precision.

Sustained non-pulsatile GnRH exposure can reduce pituitary responsiveness.

But that does not mean every imperfectly spaced native Gonadorelin bolus reproduces the pharmacology of long-acting depot GnRH agonists.

Native Gonadorelin is rapidly cleared, with human IV data showing approximately:

2-10 minutes initial half-life and:

10-40 minutes terminal half-life.

An occasional large bolus may be poorly designed as replacement therapy.

That does not automatically make it equivalent to sustained leuprolide exposure.

The regulatory distinction is equally important.

Historical FDA-approved human Gonadorelin: Yes.

Currently marketed U.S. human product identified in this review: No.

Modern research vial automatically FDA approved because the molecule once had an NDA: No.

Gonadorelin acetate on the current posted 503B Category 1 list: Yes.

503B Category 1 = approval: No.

The safety section should stay grounded in actual historical human findings: injection-site reactions, thrombophlebitis, headache, nausea, rare antibodies/anaphylaxis, multiple pregnancy and rare OHSS.

The speculative or overbroad material around universal PCOS contraindication, universal pregnancy prohibition, “chemical castration,” testicular ache, TRT mood troughs, nitrates and generic cardiovascular screening should come out or be reclassified.

The final evidence hierarchy should remain:

historical FDA-approved Gonadorelin therapy EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 30

separate from:

true human pulsatile pump therapy separate from:

modern randomized pulsatile pump research separate from:

modern female FHA outcomes separate from:

male CHH pump therapy separate from:

single diagnostic Gonadorelin exposure separate from:

TRT-clinic intermittent use separate from:

community 50-200 mcg bolus schedules.

The important discovery is not that Gonadorelin lacks evidence. It is that the strongest human evidence supports a fundamentally different exposure pattern from the protocol currently circulating in peptide communities.

The clean Navaro conclusion is:

Gonadorelin has strong human evidence when used to reproduce missing hypothalamic GnRH signaling through genuinely pulsatile administration. Modern intermittent SC bolus protocols are different exposures with a much weaker evidence base.

Independent evidence

Strong historical human evidence for specific uses

Intermittent community SC injections are not equivalent to physiologic pulsatile treatment.

Storage

Formulation / Stability

Historical Gonadorelin pharmaceutical products had standardized formulation and delivery systems.

Current peptide-vial Gonadorelin does not automatically inherit those stability specifications.

Historical Lutrepulse was supplied as lyophilized Gonadorelin acetate and reconstituted for programmable pump use according to the product’s own instructions.

Production wording Storage, dilution and beyond-use dating should be tied to the exact Gonadorelin formulation. Historical Lutrepulse or Factrel formulation data should not automatically be transferred to a modern compounded/research vial.

Do Not Apply Veterinary Product Rules

Current U.S. product databases contain veterinary Gonadorelin products, including cattle formulations with their own preservative systems and post-puncture instructions.

Those veterinary instructions do not establish stability or dosing for compounded human peptide vials.

Contraindications & cautions

Safety at a Glance

Gonadorelin has considerably more human safety experience than many experimental peptides.

But safety depends on:

• indication

• dose

• route

• sex

• ovarian status

• endocrine disease

• exposure pattern

Historical Human Adverse Findings

Documented historical findings include:

• injection-site irritation

• superficial thrombophlebitis

• catheter/infusion-site problems

• headache

• nausea

• abdominal symptoms

• increased menstrual bleeding in fertility contexts

Adverse reactions in historical fertility programs were usually related to parenteral administration and reproductive treatment context.

Antibody Formation

Rare anti-GnRH antibodies have been documented during prolonged exposure.

Classification Rare documented human immunogenicity not:

common expected side effect

Hypersensitivity / Anaphylaxis

Rare hypersensitivity and anaphylactic reactions have been reported with gonadorelin preparations.

Classification Rare documented human safety signal

Ovarian Hyperstimulation Syndrome

OHSS is possible during ovulation induction.

However , historical pulsatile GnRH data describe OHSS as rare, generally reported at less than 1% in the historical treatment experience.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 16

Production correction Do not say:

“OHSS risk is comparable to exogenous FSH.”

That is too strong.

Better wording OHSS is a recognized fertility-treatment risk, but historical pulsatile GnRH programs reported it infrequently and generally less prominently than gonadotropin-based ovarian stimulation.

Multiple Pregnancy

Multiple pregnancy is possible.

Careful dose control and ultrasound monitoring were historically part of fertility treatment because more than one follicle can develop.

Classification Established fertility-treatment risk

Pregnancy

Pregnancy should not be described as a universal gonadorelin contraindication.

Historical Lutrepulse therapy was sometimes continued after induced ovulation for luteal support.

Limited first-trimester historical exposure did not identify an apparent gonadorelin-attributable fetal signal in the available follow-up.

Correct wording Pregnancy is not a general indication for Gonadorelin. Historical Lutrepulse therapy was sometimes continued briefly after induced ovulation for luteal support. This should not be generalized to elective Gonadorelin use during pregnancy.

Breastfeeding

Historical labeling stated that it was unknown whether gonadorelin was excreted in human milk and there was no established nursing indication.

Classification Use not established

Pituitary Disease

Pituitary disease is clinically relevant because Gonadorelin requires pituitary responsiveness.

But:

“pituitary adenoma = absolute contraindication” EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 17

is too broad.

Better wording Known pituitary disease can alter Gonadorelin response and requires condition-specific endocrine evaluation. Historical fertility labeling included conditions such as pituitary prolactinoma when pregnancy itself could worsen the disease.

GnRH-Receptor Mutations

A severe loss-of-function mutation in:

GnRHR can make exogenous GnRH ineffective because the pituitary cannot respond normally.

Better classification Predictor or mechanism-based cause of nonresponse not:

formal contraindication

PCOS

PCOS should not be listed as a universal gonadorelin contraindication.

PCOS is a distinct cause of anovulation and is not the classic hypothalamic-GnRH-deficiency indication for pulsatile Gonadorelin.

Verdict Not the established indication rather than:

universal absolute contraindication

Hormone-Sensitive Cancer

Increasing LH/FSH and downstream sex-steroid production can be relevant in hormone-sensitive disease.

Classification Important disease-specific clinical caution Do not present a universal modern FDA contraindication unless tied to an exact product label and indication.

“Testicular Ache”

The live page lists testicular sensitivity/ache with a mechanistic explanation involving Leydig/Sertoli stimulation.

I did not identify a strong clinical source establishing this as a characteristic Gonadorelin adverse effect.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 18

Verdict Remove from the main evidence-based adverse-effect list.

If retained:

Community-reported / causality uncertain

Mood and Libido Fluctuations

Do not claim that predictable mood/libido troughs are established consequences of “testosterone oscillations between doses.”

If included:

Subjective symptoms may occur during changing reproductive-hormone states, but a predictable Gonadorelin-specific trough syndrome has not been established.

Priapism

Rare priapism appears in historical male Gonadorelin safety information.

This can remain if labeled clearly as:

rare historical human report rather than being mixed into generic erectile-dysfunction template language.

Remove Generic Sexual-Health Boilerplate

The live logic check contains cardiovascular disease, nitrates and generic sexual-health filters that are not appropriate central Gonadorelin screening questions.

Remove

• generic nitrate warning

• generic vascular-response framing

• unrelated erectile-dysfunction boilerplate

• metabolic/mitochondrial monitoring boilerplate

Replace with

• hypothalamic versus pituitary versus gonadal defect

• pituitary responsiveness

• fertility goal

• ovarian response where applicable

• pregnancy context

• renal function

• pulse interval

• exact formulation/product quality

Commonly Paired With

hCG Gonadorelin and hCG act at different levels of the reproductive axis.

EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 19

Gonadorelin Pituitary:

GnRHR -> LH/FSH hCG Testis:

LH/CG receptor -> Leydig-cell stimulation Classification Mechanistically distinct reproductive therapies not:

proven interchangeable or automatically synergistic

Gonadotropins / FSH / hMG

In reproductive medicine, pulsatile GnRH and exogenous gonadotropin therapy are alternative or conditionspecific approaches to supplying missing reproductive signals.

One restores the upstream pituitary command.

The other supplies downstream gonadotropin activity.

These are formal endocrine regimens, not generic peptide “stacks.”

Kisspeptin

Kisspeptin acts one level higher in the reproductive axis, stimulating endogenous GnRH neurons.

Gonadorelin directly replaces the GnRH signal.

Human combination evidence No validated combination protocol established Better classification Mechanistically related / potentially redundant upstream-axis combination not:

proven synergy

Testosterone

If exogenous testosterone is already suppressing the HPG axis, Gonadorelin should not be described as a proven way to “keep the pituitary active.”

Evidence classification Common off-label / clinic combination Controlled fertility-preservation evidence: Not established EDUCATIONAL RESEARCH SUMMARY | GONADORELIN | AUDITED EVIDENCE STANDARD Page 20

11. Contraindications

Reclassify:

• pituitary adenoma: condition-specific endocrine caution, not universal blanket prohibition

• PCOS: not the established indication, not universal contraindication

• GnRHR mutation: likely nonresponse, not formal contraindication

• pregnancy: indication/treatment-stage specific

• hormone-sensitive malignancy: important disease-specific caution

12. OHSS

Remove the statement that risk is comparable to exogenous FSH.

13. Kisspeptin

Replace:

Reported synergistic with:

Mechanistically related / commonly paired; controlled combination efficacy unproven

Possible side effects

The supplied chapter does not establish a separate side-effect list for this compound.

Compatibility

Same-Syringe Compatibility

There is no universal evidence supporting physical or chemical compatibility between Gonadorelin and:

• hCG

• testosterone

• Kisspeptin

• FSH

• other injectable peptides

Verdict Concurrent use does not establish same-syringe compatibility.
